MAHA Credential Book

All Association teams at the 10U level and above are required to maintain an organized team book for MAHA Book Check.

You will work with the Association Registrar to receive your team book and make sure you have the required paperwork. Throughout the season, keep your book organized and up to date.

Your team book should have:

• Official Team Roster — Printed after January 1, 2027.
• Credential Verification Sheet — Printed after January 1, 2027.
• Official Game Scoresheets — A scoresheet for every game played.
• Game Log — Keep your game log current and organized with your scoresheets.
• Player Participation — Make sure player participation is properly recorded and matches your game records.
• Consent to Treat Forms — Required for every participant for 14U AA teams only.
• Other Required MAHA/District Paperwork — Include any additional paperwork required for your team or current season.

Bring your organized team book to one of the designated Book Check locations during the scheduled times. The person bringing the book does not have to be a coach, it can be a team manager, parent, or another responsible team representative.

If you cannot make one of the scheduled Book Check times, contact your District Director ahead of the deadline to make other arrangements.
